Why Are My Thigh Veins Visible?

Visible veins on the thighs are among the most common vein concerns, particularly in women. These can range from fine spider veins covering the outer thigh to prominent, twisted varicose veins running along the inner thigh. Types of Visible Veins on Thighs

1. Varicose Veins on the Inner Thigh (GSV Incompetence)

The Great Saphenous Vein (GSV) runs along the inner thigh from the groin to the ankle. When the valves in the GSV fail (venous reflux), the vein dilates and becomes visible as a twisted, bulging cord on the inner thigh. This is the most medically significant type of thigh vein and should be treated with endovenous laser ablation (EVLA) or MOCA.

2. Reticular Veins on the Thigh

Reticular veins are medium-sized (2-3mm) blue-green veins visible in a branching pattern under the thigh skin. They are larger than spider veins but not as prominent as varicose veins. They often feed networks of spider veins and should be treated with sclerotherapy for best cosmetic results.

3. Spider Veins on the Thigh

Spider veins on the outer and inner thigh appear as fine red, blue or purple web patterns. They are extremely common in women and are typically a cosmetic concern. Treatment with laser or sclerotherapy gives excellent results.

What Causes Thigh Veins to Become Visible?

• Venous reflux in GSV: Most important cause of prominent inner thigh varicose veins
• Genetics: Strong family history in most patients with varicose veins
• Hormonal factors: Oestrogen weakens vein walls — common after pregnancy or with oral contraceptives
• Prolonged standing: Increases pressure in leg veins over time
• Obesity: Increased abdominal pressure impairs venous drainage from legs
• Sun exposure: Causes surface spider veins on the outer thigh

Treatment Options for Thigh Veins

For Varicose Veins on Inner Thigh:

• EVLA (Endovenous Laser Ablation) — laser fibre inserted into GSV under ultrasound; vein permanently closed; same day home
• MOCA (VenaSeal/Clarivein) — mechanochemical closure without heat; no risk of thermal skin injury
• Foam Sclerotherapy — suitable for smaller varicose tributaries of the GSV

For Reticular & Spider Veins on Thighs:

• Sclerotherapy — fine needle injection of sclerosant into reticular and spider veins
