What Are Spider Veins on Calves?
Spider veins on calves are tiny, dilated blood vessels that appear just under the surface of the skin, usually forming red, blue or purple web-like patterns on the calf area. Medically called telangiectasias or thread veins, they are extremely common — affecting nearly 40% of women and 25% of men at some point in their lives.
While spider veins are primarily a cosmetic concern, they can sometimes cause mild burning, itching or heaviness in the legs. More importantly, spider veins on the calves are sometimes an indicator of underlying venous reflux, which should be evaluated before treatment.
Why Do Spider Veins Appear on Calves?
The calf is one of the most common locations for spider veins because it is the area subjected to the highest venous pressure when standing. Causes include:
- Increased venous pressure from varicose veins or venous reflux feeding the spider veins
- Prolonged standing as in the profession of nurses, teachers, shopkeepers and security staff
- Genetics: Strong hereditary component — if a parent has spider veins, there is a high chance of developing them
- Hormonal factors: Oestrogen exposure through pregnancy, contraceptive pills or HRT
- Sun exposure: UV damage can cause or worsen spider veins, especially on the thigh and calf
- Trauma or injury to the calf can sometimes trigger local spider vein formation
- Sedentary lifestyle: Reduced calf muscle pump activity allows venous pressure to build

Treatment Options for Spider Veins on Calves
Laser Treatment for Spider Veins
A surface laser (Nd:YAG) is applied over the spider veins on the calves. The light energy is absorbed by haemoglobin in the blood, generating heat that closes the tiny vessel. The skin over the vein remains undamaged. Treatment takes 15-30 minutes per session and 2-4 sessions are usually needed for complete clearance. No injections, no downtime.
Microsclerotherapy
A very fine needle is used to inject a sclerosant solution directly into the spider vein. The chemical damages the inner lining of the vein, causing it to collapse and fade over 4-8 weeks. This is the most effective treatment for spider veins on calves, with a high clearance rate. Multiple veins can be treated in one session.
Combination Therapy
For best results, especially when spider veins on the calf are fed by larger varicose veins, we first treat the underlying varicose veins with laser or MOCA, then address spider veins with sclerotherapy or surface laser. This prevents rapid recurrence of spider veins after treatment.
