What Problems Can Varicose Veins Cause?

Varicose veins are often dismissed as a cosmetic concern, but untreated problems caused by varicose veins can become seriously debilitating. 2. Skin Discoloration (Hyperpigmentation)

Chronic high venous pressure causes red blood cells to leak from capillaries into skin tissue. As the haemoglobin breaks down, it leaves a brown pigment (haemosiderin) staining the skin permanently — particularly around the ankle area. This is called venous hyperpigmentation and indicates advancing venous disease.

3. Lipodermatosclerosis (Skin Hardening)

Ongoing inflammation and fibrosis from chronic venous disease hardens the fatty tissue just under the skin — particularly above the ankle. This condition (lipodermatosclerosis) causes the lower leg to become hardened, tender and sometimes narrowed at the ankle, giving it an "inverted wine bottle" appearance. It greatly impairs healing of any wounds in the area.

4. Venous Eczema

Itchy, inflamed, scaly skin patches (eczema) near varicose veins are caused by immune reactions to leaking venous fluid. Venous eczema is difficult to treat with creams alone — treating the underlying varicose veins is essential to resolution.

5. Superficial Thrombophlebitis

Blood clots forming inside varicose veins cause painful, red, tender, cord-like segments along the vein. Called superficial thrombophlebitis, this typically resolves with anti-inflammatory treatment, but can spread to deeper veins (DVT) and requires monitoring.

6. Deep Vein Thrombosis (DVT)

In some cases, clots from varicose veins propagate into the deep venous system, causing DVT — a potentially life-threatening condition if the clot breaks off and travels to the lungs (pulmonary embolism). Patients with varicose veins are at higher risk of DVT, especially during prolonged bed rest, surgery or long-haul travel.

7. Spontaneous Bleeding

Large, thin-walled varicose veins near the surface of the skin can rupture and bleed — sometimes profusely and without any injury. This is an emergency that requires immediate elevation and compression, followed by urgent treatment of the varicose veins.

8. Venous Leg Ulcers

The most severe complication of untreated varicose veins is the development of venous leg ulcers — open wounds, usually near the inner ankle, that do not heal for months or years. Venous ulcers affect quality of life profoundly and often only heal after the underlying venous disease is properly treated. They are entirely preventable with early varicose vein treatment.

  • Patients who have had varicose veins for many years without treatment
  • People with advanced CEAP stage C4, C5 or C6 venous disease
  • Those with significant venous reflux on Doppler scanning
  • Elderly patients with associated obesity and reduced mobility
