"Doctor, are these spider veins or varicose veins?" — it is one of the most common questions we hear at Dr. Raja's Clinic. The two conditions look superficially similar, but they are actually very different in their root cause, symptoms, danger level and treatment.
This guide gives you a clear, doctor-written comparison so you can understand what you are dealing with before your consultation.
Quick side-by-side comparison

What are varicose veins?
Varicose veins are large, bulging, twisted veins that you can both see and feel. They sit just below the skin and develop when the one-way valves inside the deep saphenous vein system fail. Blood that should flow upward against gravity instead pools backward — known as venous reflux — causing pressure to build up and the surface veins to swell over months and years.

What are spider veins?
Spider veins (medically telangiectasias) are tiny, thin red or purple lines that branch out across the skin like a spider web — usually on the thighs, calves, ankles or face. Each individual vein is less than 1 mm wide and lies inside the skin, not below it.
They are caused by capillary-level dilatation, usually triggered by:
- Genetics (most common)
- Hormonal changes (puberty, pregnancy, contraceptive pills, menopause)
- Sun damage (face)
- Long hours of standing
- Sometimes secondary to underlying varicose vein reflux (in 20% of cases)
Are spider veins ever dangerous?
By themselves, no — spider veins are almost always purely cosmetic. However, they can be a warning sign of underlying varicose vein disease in 1 out of every 5 patients. That is why we recommend a quick duplex Doppler ultrasound for any spider vein patient who also has:
- Leg heaviness or aching at the end of the day
- Swelling around the ankles
- Restless legs at night
- A family history of varicose veins or DVT
- Spider veins concentrated below the knee on the inner side of the calf
If reflux is found, the underlying varicose vein needs treatment first — otherwise the spider veins will keep coming back even after sclerotherapy.
Treatment side-by-side
For varicose veins (large, bulging veins)
- EVLT laser — gold-standard, ₹45,000–₹95,000 per leg
- RFA — slightly less bruising, ₹65,000–₹1,10,000 per leg
- VenaSeal glue — no anaesthesia, no stockings, ₹1,40,000–₹1,90,000 per leg
- MOCA — newer mechanochemical option, ₹1,10,000–₹1,50,000
For spider veins (small, web-like veins)
- Foam sclerotherapy — ₹6,000–₹12,000 per session, usually 2–4 sessions needed
- Surface laser (Nd:YAG 1064 nm) — for facial spider veins or stubborn red telangiectasias, ₹4,000–₹10,000 per session
Critically: never inject spider veins if there is untreated underlying reflux. The procedure will fail. Always insist on a Doppler scan before sclerotherapy.
Can both occur together?
Yes — and very commonly. Up to 30% of patients with visible varicose veins also have associated spider veins. The correct treatment sequence is:
- Close the underlying refluxing saphenous vein (EVLT / RFA / VenaSeal)
- Wait 6 weeks for healing
- Treat residual spider veins with foam sclerotherapy in 2–3 sessions
This sequence gives the best long-term cosmetic and clinical result. Read more on problems caused by untreated varicose veins.
Conclusion
Varicose veins and spider veins are different conditions that need different treatments — and getting the diagnosis right makes all the difference between a successful, cost-effective outcome and a frustrating cycle of recurrence. Frequently Asked Questions
What is the main difference between varicose veins and spider veins?
Size and depth. Varicose veins are 3+ mm wide, bulging and palpable below the skin, caused by deep vein valve failure. Spider veins are under 1 mm wide, flat, web-like and lie within the skin — caused by tiny capillary dilatation.
Are spider veins dangerous?
Spider veins by themselves are almost always cosmetic and harmless. However in 20% of cases they are a warning sign of underlying varicose vein reflux, which is why a duplex Doppler scan is recommended before treatment.
Can sclerotherapy treat both varicose and spider veins?
Foam sclerotherapy is excellent for spider veins and small varicosities, but large bulging varicose veins (>5 mm) caused by saphenous reflux need EVLT, RFA or VenaSeal first. Sclerotherapy alone will fail.
Why do my spider veins keep coming back after treatment?
The most common reason is untreated underlying saphenous vein reflux. Always get a duplex Doppler scan before sclerotherapy — if reflux is found, treat the deep vein first, then the spider veins will respond properly.
Is spider vein treatment covered by insurance?
No — spider vein treatment is classified as cosmetic and is generally not covered by Indian insurance. Varicose vein treatment with documented reflux IS covered.
How much does it cost to treat spider veins?
Foam sclerotherapy costs ₹6,000–₹12,000 per session in Punjab. Most patients need 2–4 sessions for complete clearance.
